All required laboratory tests; immunity checks for Hepatitis B, MMR, and/or Varicella (ages 18+ only, if relevant); required vaccines†; physical examination and completion of USCIS Form I-693.
†Although we are in-network with most major insurers, some clients may need to receive their vaccines at the pharmacy to make use of their policy benefits. We do not work with most private, short-term or traveler's policies (e.g., US Health Group, IMG).
Self-Pay — Children
Ages 0–18
$300*
All required laboratory tests, physical examination, and completion of USCIS Form I-693.
Vaccines, if missing, are available at our Oakland Park office for $20 each via the Vaccines for Children (VFC) Program — or for free at the health department.
Self-Pay — Adults
Ages 19+
$350
All required laboratory tests; immunity checks for Hepatitis B, MMR, and/or Varicella, if relevant; physical examination, and completion of Form I-693.
Vaccines, if needed, are available in-office for an additional fee.
** Aventura / North Miami (Affiliate) affiliate location charges $350 for all clients, regardless of age or insurance status.
