Attend your appointment
- Your Civil Surgeon performs a physical examinationAssessment of medical history, vitals and bodily systems
- Specimen collection for transmittable disease testingIncludes blood draw and/or urine sample, as applicable.Immunity titers will be performed for eligible adults.
- Administration of required vaccine boostersMany clients will receive a polio and/or tetanus shot.

Receive your laboratory results
Results typically return within 3-4 business days. We'll call to let you know if anything needs follow-up prior to us sealing Form I-693. Please monitor your texts closely to see if we left you a message. - 5
Complete any required follow-ups
Lab results and/or medical history may necessitate additional vaccination, chest x-ray, specialist evaluation, and/or antibiotic treatment. Our staff will provide you with detailed instructions for any services that must be performed outside the clinic. - 6
Pick up or receive your sealed Form I-693
Choose the option that works best for you:
- Option A — Pick up in person: Receive any outstanding vaccines not administered at the first visit and walk out with your sealed Form I-693, ready to mail to USCIS.
- Option B — Mail delivery: If all requirements are complete, we can mail your Form I-693 via USPS Priority Mail to you or your attorney for $20.
