I-693 Medical Exam Prep Checklist
Everything you should bring and know before your USCIS immigration medical exam — get the full checklist as a PDF.
What to bring to your appointment
- Government-issued photo ID (passport, driver's license, or state ID)
- Vaccination records — childhood, school, military, etc.
- List of current medications and any major medical conditions
- Payment for the exam — cash, card, or Zelle
Bring only if it applies to you
- If you've had a positive TB test: bring laboratory result and chest x-ray report
- If you have HIV: bring most recent blood work panel including CD4 count
- If you've had syphilis: bring initial laboratory result and evidence of treatment
- If you've had a DUI: bring police report and proof of any counseling
Before your appointment
- No fasting required — eat and drink normally
- Take your usual medications as prescribed
- Wear a shirt with sleeves that roll up easily
What happens during the exam
- Review of medical history and current medications
- Physical exam by USCIS-designated civil surgeon
- Blood draw to test for TB (ages 2+) and/or syphilis (ages 18–44)
- Urine specimen collection — for gonorrhea testing (ages 18–24)
- Alcohol, Tobacco and Drug Use Screening Interview
- Vaccination Review and Application of Any Required Booster Shots
After your exam
- Clients will receive a call within 3-4 business days with their results
- Some cases require follow ups which may include additional vaccinations, treatment, and/or referral to a specialist
- Clients receive a sealed Form I-693 once lab results are back and all follow-ups are complete (if applicable)
- The sealed Form I-693 is valid indefinitely unless the immigration case is denied
- You will receive an unsealed copy of your Form I-693 to keep for your personal records
